Our Psychiatric Rehabilitation Program for Adults (PRP) provides a comprehensive array of services designed to enhance the functionality and stability of individuals diagnosed with mental health illnesses, while also reducing homelessness. Our adult services focus on assisting clients in various goal domains as outlined below:
- Housing: Collaborating with clients and their families, we develop housing plans and facilitate exploration. We provide in-home or community-based sessions to address any housing needs. In the event of homelessness, we meet clients in the community and assist in connecting them to relevant services, as well as supporting them in completing necessary applications. Throughout the program, our aim is to work towards long-term financial security and independent housing in the participant's preferred community.
- Employment: We engage with clients to identify vocational interests and goals, offering guidance throughout the employment training process. This includes assistance with mock interviews, resume-building, monitoring progress towards vocational goals, and addressing any work-related issues. We also support clients in pursuing independent employment by assisting with job applications and follow-up.
- Transportation: We assist clients in identifying transportation needs and overcoming barriers to independent transportation.
- Wellness Management: We help clients recognize and address substance abuse, mental health, and physical health needs. Collaborating with treatment providers, we ensure that clients are connected to the necessary services and maintain ongoing collaboration throughout the program.
- Legal Support: We aid clients in developing a plan to prevent recidivism and collaborate with external legal support as needed.
- Family and Community Supports: We guide clients in identifying recreational interests, addressing family issues, and developing strategies for stabilization within the community. Our culturally appropriate activities and social skills training contribute to supporting family interactions.
- Education: We assist clients in identifying educational interests and goals, providing guidance throughout the educational training process and helping them access additional educational resources independently.
- Entitlements, Financial Assistance, and Budgeting: We support clients in identifying financial needs, assisting in accessing entitlements and financial assistance, and providing follow-up support for applications and recertification processes. Our field staff is trained to help clients achieve financial security through the development of budgeting plans.
- Therapy Compliance: We work closely with clients to ensure active participation in therapy and regular contact with psychiatric services, with dedicated compliance specialists to support clients in maintaining connections with mental health counseling.
- Illness Management: In our holistic approach, we assist clients in keeping up with medical appointments to address all their medical needs.
Social Skills Development Includes:
- Community integration activities
